To strengthen our newly created dementia team in Reinach, we are looking for you as
Certified Nursing Specialist HF Dementia Department (50-80%)
What awaits you:
- Day shifts from Monday to Friday including holidays
- Independent, competent and resource-oriented care of our clients and their relatives in dealing with dementia diseases in their familiar living environment
- Taking over case management for a defined client group
- Responsible for planning, controlling and monitoring the care process
- Conducting assessments and status discussions with RAI-HC
- Supporting our clients in a self-determined everyday life at home by promoting independence and quality of life
- Interdisciplinary collaboration with relatives, doctors and other services
What you bring:
- Completed training as a certified nursing specialist HF
- Professional experience and broad expertise
- Additional training in the field of dementia is an advantage (or willingness to further educate in this area)
- An independent, responsible way of working
- High empathy and sensitivity
- Willingness to travel by car
