In January 2026, the canton of Aargau will launch the innovative pilot project "Intensive Case Management" (ICM). The project, running until June 2029, is being implemented jointly by PDAG, the addiction counseling ags, and BZBplus as sponsoring organizations.
The goal of the project is to establish a specific, low-threshold, and needs-oriented case management in the addiction sector for particularly demanding and complex cases. This is intended to ensure intensive and continuous care for hard-to-reach people with addiction disorders.
The work is carried out in an inter-institutional team of professionals from social work, medicine, psychology, and addiction counseling. The case managers work outreach-oriented, networked, and resource-oriented to sustainably stabilize the living situation of the clientele. The position is limited until June 2029, with the option of extension depending on project development.
From January 1, 2026, or by arrangement, we are looking for you as
Tasks
- Recording and assessment of the individual situation of the clientele, including an initial needs analysis and social diagnostics (intake)
- Responsibility for case distribution to the case managers
- Outreach, intensive care and support of the clientele according to the case management process as needed
- Building and co-designing the new case management structure within the framework of the pilot project
- Development and implementation of work processes, workflows, and interfaces
- Continuous further development of the content and organizational orientation of the offer
- Professional and organizational management of the ICM project together with two other project managers from addiction counseling ags and BZBplus
- Overall professional leadership and coordination of the interprofessional ICM team
- Ensuring the flow of information and communication between the project team, sponsors, and project management
- Representation of the project to the professional public and network partners
- Support and control of the scientific evaluation in cooperation with the evaluation team (PDAG)
Profile
- Completed studies in social work (FH/Uni), ideally supplemented by further training in case management
- Professional experience in the social psychiatric context or in the addiction sector is an advantage
- Enjoyment of conceptual work and interest in building new services
- High social competence, solution orientation, and strong communication skills
- A confident and competent appearance in contact with clients and network partners
- Teamwork skills, sense of responsibility and quality awareness, as well as an independent and structured way of working
- Openness and motivation for outreach work in the living environment of the clientele
