Liaison Nurse - Lausanne Region Health Network
Context
The Lausanne Region Health Network is an association that brings together more than a hundred health institutions and professionals – CHUV, treatment and rehabilitation centers, nursing homes, home help and care (CMS), and doctors – as well as 61 municipalities.
With about 95 collaborators, we contribute to patient orientation and continuity of care, notably through the Regional Information and Orientation Office (BRIO). We also carry out projects aimed at improving the quality and coordination of care, mainly benefiting an elderly population weakened by one or more chronic diseases.

If so, you are the person we are looking for to strengthen our teams and thus work as a LIAISON NURSE (80% to 100%)
Mission
- Anticipate and plan the orientation of the user within the care network according to their situation, in collaboration with other professionals, taking into account their needs and the resources of the health system. Also organize short and long stays in nursing homes and collaborate on the return home with the necessary help and care
- Facilitate and coordinate continuity of care by communicating and acting as an interface/pivot between the different partners
- Inform and propose orientation options to the user, their close caregivers, and professionals based on the evolution of the situation. Activate the required services.
Profile
- Nursing diploma (HES bachelor or equivalent)
- Professional experience in hospital and community settings and good knowledge of the Vaud socio-health network
- Ability to work collaboratively with family, medical, and institutional networks, in an innovative context, with strong listening, empathy, and negotiation skills
- Proficiency in common office applications and ease with administrative work
- Analytical, synthesis, and creativity skills, as well as organizational and priority management abilities.
