Your task
• Carrying out care services for clients in their homes according to care planning and medical orders
• Documenting care in the progress report
• Adjusting measures in the care plan in collaboration with the case manager
• Goal- and solution-oriented collaboration within the interdisciplinary team
Your profile
• Completed training as a Health Specialist EFZ
• Flexibility and willingness for assignments with variable working hours
• Enjoyment of independent work combined with active teamwork
• Proficient IT skills and a valid category B driving license
