Your Tasks
Advising, supporting, and accompanying patients and their relatives in coping with personal, social, and/or financial problems related to illness and hospital stay
Independently assessing the personal and social situation of our patients as well as comprehensive discharge management
Initiating adult protection measures
Cooperating with external agencies such as Spitex, retirement and nursing homes, counselling centres, and social services
Possibly participating in projects with internal and external cooperation partners

Your Profile
Bachelor's degree in "Social Work" or a comparable qualification
Independent, reliable, and networked working style
Team-oriented, empathetic personality with high resilience, assertiveness, and strong negotiation skills
Very good PC and German skills; good knowledge of social insurance law
Foreign language skills, project experience, and willingness to engage in the professional development of the team are very welcome
