Your Tasks
- Advising patients and relatives on medical and occupational rehabilitation
- Advice on financial matters (e.g. application for care level)
- Preparation for discharge and arrangement of outpatient assistance (e.g. home nursing care)
- Coordination of aid supply
- Communication with relatives, carers, cost bearers, aid suppliers, and government agencies
Your Profile
- State-recognised diploma or bachelor’s degree
- IT skills
- Social competence and communication skills
- Ability for professional and multiprofessional exchange
- Good knowledge of social legislation
