Your tasks
- Organisation and mediation of outpatient and inpatient aftercare services, e.g. assisted living, care services, self-help groups and care facilities
- Application and coordination of benefits with health and nursing care insurance funds as well as other cost bearers
- Advice on social law issues, especially on health and nursing care insurance, severely disabled and guardianship law and powers of attorney
- Support in social reintegration, initiation of rehabilitation measures and long-term therapies as well as securing the financial/material livelihood
- Establishment and maintenance of regional care networks as well as close cooperation with external institutions
- Active participation in structured discharge management, attendance at ward rounds and multiprofessional team meetings
- Independent conduct of social therapeutic counselling sessions
Your profile
- Completed degree in Social Work or Social Pedagogy (Diploma or Bachelor)
- Several years of professional experience in psychosocial work, ideally with people with mental illnesses or addiction disorders
- High motivation for work in the psychiatric environment
- Sound knowledge of social law and relevant social legislation (e.g. SGB)
- Independent, structured and responsible way of working
- Strong communication skills and ability to work in a team
- Willingness to cooperate and interdisciplinary thinking in the multiprofessional team
