About the job
What you will achieve with us
- You will independently advise our patients and support them and their support system in difficult life situations related to illness and hospital stay
- You will mediate and coordinate external services, advise patients and their relatives on inpatient follow-up solutions (nursing and care homes, rehabilitation and convalescence) as well as outpatient care and support services and organise these
- You will perform administrative tasks and maintain patient records, progress notes and statistics
- You will actively participate in the interdisciplinary exchange on the acute geriatric rehabilitation ward
- You will organise and lead interdisciplinary family discussions.
- You are proficient in social insurance matters related to illness and accident and identify financial resources
- You coordinate the networking of patients with Pro Senectute, municipal social services, the child and adult protection authority (KESB) and other third parties
What you bring with you
- You have a completed Bachelor’s degree in Social Work FH BSc/B.A., a Bachelor’s degree in Social Pedagogy HF or an equivalent qualification and/or hold a CAS in Case Management and ideally have experience in clinical social work
- You approach people in challenging health situations with empathy and appreciation and demonstrate skill in communication primarily with older patients
- You have good knowledge of social law issues
- You think and act in a networked way in the sense of an interdisciplinary team in the clinical field and enjoy interdisciplinary cooperation
- You have the ability to find creative solutions to different requirements while focusing on the needs of the addressees
- You are a resilient, flexible personality accustomed to working independently
- You are proactive, sociable and take on internal team tasks
- You are characterised by a structured way of working and keep an overview even in busy times
