Spitex Am Alten Rhein currently employs 22 staff members in the field of home care nursing. Our services are available to all residents of the political municipalities of Lutzenberg, Rheineck, St. Margrethen, and Thal who, due to illness, accident, disability, age-related ailments, maternity, or similar, require support.
We are looking for an immediate or agreed start for a
Certified Nursing Specialist HF/FH 40%
Your tertiary-level nursing education forms the foundation for competent assignments in the outpatient area of Spitex.
What are the general responsibilities?
- Individual, person-centred care and support of our clients according to independent case management and corresponding care plans (Perigon, interRAI HC)
- Electronic documentation of progress, directly on site using the company mobile phone, as well as at the base for administrative tasks related to your case management
- Collaboration with other case-managing nursing specialists HF/FH and the operations management
- Multiprofessional collaboration with doctors as well as other service providers and advisory centres
- Consultation in complex care situations for chronically ill people and cooperation with caregiving relatives
- Organisation or leadership of round-table discussions and client meetings
- Independent management of daily rounds with the company car
