Social Worker or Nurse in the Community Psychiatry Service
The Community Psychiatry Service (PCO) is looking for a social worker or a nurse for the mobile psychiatry team. This is a fixed-term contract (FTC) of 5 months starting March 1, 2026, or as agreed.
Context
The Lausanne University Hospital (CHUV) is one of the 5 Swiss university hospitals. Thanks to its collaboration with the Faculty of Biology and Medicine of the University of Lausanne and EPFL, CHUV plays a leading role in medical care, medical research, and education.
The Department of Psychiatry consists of 10 services addressing the three stages of life. It covers the Central, Western, and Northern psychiatric sectors of the canton. Its mission is the comprehensive and multidisciplinary care of children, adolescents, adults, and elderly people suffering from mental disorders. Caregivers provide acute care but also offer outpatient follow-ups and support in patients' usual living environments.
The Community Psychiatry Service (PCO) aims to develop a social psychiatry hub oriented towards the community. It brings together various units with a community vocation.
The activities of the mobile psychiatry service are organized around four axes:
- Intensive Follow-up in the Environment (SIM)
- Transition Case Management (CMT)
- Psy&migrants
- Social support for practicing physicians
Mission
As a social worker or nurse in the Intensive Follow-up in the Environment (SIM), your main missions will be as follows:
- You ensure a transition follow-up of case-management type in the environment for people with significant mental disorders while promoting their entry into care
- You facilitate access to the care network and encourage patient engagement by integrating their primary and secondary networks
- You meet beneficiaries, assess their difficulties in collaboration with a psychiatrist, organize additional investigations and/or referral to appropriate care
- You conduct psychosocial assessments and follow-up of patients with social issues.
- You share your expertise with the multidisciplinary team
- You inform, guide, and/or support the patient and their relatives in concrete steps: administrative, professional, legal, and relational
- You investigate to assess the role and skills of actors present in patients' primary and secondary social networks
- You work in collaboration with the multidisciplinary team of the mobile psychiatry section, with partners of the Community Psychiatry Service and the Vaud medico-social network
- You undertake liaison interventions (case discussions, meetings with potential patients) with partner institutions in the canton
- You actively participate in the projects of the Community Psychiatry Service.
Profile
- For social workers, you hold a Bachelor of Arts HES-SO in Social Work or an equivalent title - mandatory SEFRI recognition for foreign diplomas
- For nurses, you hold a HES nursing diploma or an equivalent title as well as Red Cross recognition for foreign diploma holders, imperative
- You have experience with people presenting mental disorders
- You have good knowledge of the Vaud socio-health network, medico-social institutions, administrative and legal structures, and accommodation facilities
- You have good autonomy and a creative and initiative spirit
- You possess good analytical and synthesis skills
- You know how to listen actively and communicate effectively
- You have a spirit of mutual aid and collaboration and demonstrate openness and flexibility
- You hold a driving license and a car (essential).
Depending on the profile, the salary scale will be between level 8 and level 9.
