- Lead the care and support process for assigned clients.
- Manage casework using the InterRai HC assessment tool.
- Ensure implementation of nursing diagnostic processes.
Skills
- Diploma in nursing (HF, DN II, AKP) or recognized tertiary education.
- Experience with Perigon and InterRai HC is a plus.
- Strong social and professional competence required.

Registered Nurse HF, Workload 30 - 50%
Your tasks
- Management of the nursing and care process for the clients assigned to you
- Taking over case management with the assessment tool InterRai HC
- Responsibility for the implementation of the nursing diagnostic process
- Supporting staff in specialist questions and complex wound treatments
- Co-responsibility for ensuring the required quality of care
- Interdisciplinary collaboration with relatives, doctors and other services
Your profile
- Diploma Registered Nurse HF, DN II, AKP or SRK-recognised tertiary level qualification
- Experience with Perigon and InterRai HC is an advantage
- High social and professional competence
- Very good oral and written German skills
- Driving licence category B
