Your versatile field of activity in nursing and counseling

• Ensuring needs-oriented, appropriate, and professional nursing and counseling in the home environment, including the social network. The goal is to stabilize the home situation and adequately support close relatives.
• Responsibility for the nursing process and case management, including initial assessment and reassessment, recognizing changes, adjusting care planning, and evaluating implemented measures.
• Coordination of services to promote efficient collaboration among all parties involved in nursing and care.
• Promotion of the maintenance and recovery of individual abilities, taking into account their resources and social environment.
• Taking on tasks in the practical training of students and apprentices, promoting knowledge and experience transfer within the team.

What you bring with you

• Degree as a nursing specialist HF/FH 
• Solid theoretical and practical knowledge and experience in nursing 
• Willingness to take responsibility and contribute to shaping outpatient care
• Enjoyment in working with people and diverse nursing tasks
• Organizational talent and empathetic communication
• Driver’s license category B
