Your Tasks
- Needs assessments with clients and relatives
- Ensuring the care process
- Leading case discussions
- Performing basic and treatment care for clients
- Supporting the PDL in the introduction and training of new employees
- Quality assurance through practical support for caregivers
- Ensuring the flow of information with external partners (doctors, health insurance companies)
What you bring
- Tertiary education as a nurse HF/FH/AKP
- InterRAI-HC knowledge
- Experience in the field is an advantage
- You are flexible and can adapt to changing situations
- Spitex experience is an advantage
- Driver's license required
