The Dielsdorf Health Centre is a modern geriatric competence centre with around 500 employees from 35 nations. Our services range from advisory services for older people and their relatives to general and specialised long-term care as well as home care and support. With over 200 beds, the inpatient general long-term care offering at the Dielsdorf site is complemented by specialised treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.

• Accompanying and caring for residents with a focus on palliative care
• Supporting the implementation of admission, discharge, and transfer processes
• Maintaining care documentation and creating care plans
• Supporting palliative care and contributing to gerontopsychiatric or rehabilitation processes
• Collaboration within the interdisciplinary team (nursing, medical service, therapists, as well as counselling and social services)
• Assuming daily responsibility

• Completed training as a Health Specialist EFZ or an equivalent with SRK recognition
• Good German language skills in spoken and written form (at least level B1)
• High social, methodological, and professional competence
• Up-to-date IT skills
• Independent and responsible working style
