The cross-sectional area of Nursing, Education and Social Services is composed as follows: Competence Centre Education, Personnel Pool flex26, Nursing Development, De-escalation Management, Social Services and Central Admission. The Competence Centre Education supports staff with and in training, further education and continuing education, thus making an important contribution to ensuring professional and high-quality nursing care. Nursing quality is also a central concern of Nursing Development, which, together with the specialist managers, leads cross-departmental projects based on evidence-based knowledge. Thanks to our own personnel pool flex26, we succeed in compensating short-term vacancies/absences quickly and professionally with PDAG internal nursing staff. De-escalation management promotes the treatment team’s competence in de-escalating milieu design, advises and supports employees before and after threatening situations. It develops interdisciplinary training concepts and conducts further training on this topic. Social Services is an important part of the interprofessional treatment offer. It supports and promotes patients in social and professional integration with the aim of regaining their independence in everyday life. Central Admission is the first point of contact for external referrers, assists with questions about our services, accepts referrals and triages them.
Profile
- Bachelor of Science / Arts from a University of Applied Sciences for Social Work
- Professional experience in working with older people or a comparable environment
- High presentation skills, service orientation, initiative, flexibility, independence and resilience
- Interested and empathetic personality with joy and motivation for networked working
- Your strengths include a positive view of humanity, social skills, solution orientation as well as a healthy dose of humour and pragmatism
- You are a team player with a high sense of responsibility and quality awareness and are used to setting priorities and working independently
- You place dignity, self-determination and social participation of older people at the centre of your work and can recognise and professionally deal with the associated opportunities and limits.
Tasks
- You advise patients and their relatives regarding suitable forms of accommodation and daily structure as well as social insurance law, financial and adult protection law issues. In doing so, you maintain close contact with the involved specialists.
- You promptly record the needs of newly admitted patients and their relatives and jointly seek suitable and sustainable follow-up solutions for discharge from the clinic. You orient yourself to legal and economic framework conditions and shape the available scope for action in the interest of the patients.
- You contribute your expertise to the interprofessional treatment team, namely from medical, psychological and nursing specialists, and actively support the achievement of treatment goals.
- You participate in reports and case discussions
