ASSC – ASSISTANT IN COMMUNITY CARE AND HEALTH AT 60-80% PERMANENT CONTRACT
Mission:
After a period of integration and specific training on the diversity of situations and the population supported by our CMS, you will work autonomously with beneficiaries in their homes.
In close collaboration with the multidisciplinary team and the situation referents, you will play a central role in the implementation and evaluation of care objectives.
Your main tasks:
- Carry out delegated medical care, basic care and household help in compliance with prescriptions and individualised needs.
- Observe, assess and report changes in the health status of beneficiaries in order to adapt care.
- Support relatives and families, and contribute to interdisciplinary coordination.
- Actively participate in the continuous improvement of care quality and the sharing of practices within the team.
This position requires great autonomy, analytical and evaluative skills, as well as a strong interest in home care and community health.

- Comfortable with IT tools (patient file management, care planning, quality indicator monitoring).
- Driving licence and vehicle essential
- For foreign diplomas, Red Cross recognition is required or in process.
