About the job
The Dielsdorf Health Centre is a modern geriatric competence centre with around 500 employees from 35 nations. Our services range from advisory support for older people and their relatives to general and specialised long-term care, as well as home care and support. As the largest public Spitex in the Dielsdorf district, we provide outpatient services for our nine contract municipalities. With our comprehensive care offerings, we enable people to remain in their familiar homes for as long as possible despite health limitations. In the future, it is planned to establish a night Spitex service.
- Care, support and advice for our clients at home
- Organisation and planning of your daytime work from Monday to Friday
- Responsibility for conducting assessments with Inter RAI-CMH, psychosocial care and support for chronically and mentally ill people, as well as conducting conversations and recovery-oriented accompaniment of therapeutic processes
- Intensive exchange, also beyond the boundaries of the Dielsdorf Health Centre, with relatives, social workers, the Child and Adult Protection Authority (KESB), specialist agencies, doctors, etc.
- Active participation in the development and further advancement of the psychiatry Spitex service
- Completed training as a Dipl. Registered Nurse DN II/HF/FH
- Several years of professional experience in the field of psychiatry
- High social, methodological and professional competence
- Very good German language skills, both spoken and written
- Up-to-date IT skills (MS Office, Perigon)
- Driving licence category B, own car preferred
- Independent and responsible working style
