Your tasks
- Organisation of the entire admission or transfer process as well as conducting the necessary work and team meetings
- Consultation with specialists on financial and social law matters
- Cooperation with authorities to clarify guardianship
- Advice on spa, rehabilitation or care stays
- Information about outpatient services
- Obtaining cost credits from health insurers for spa contributions and rehabilitations
- Responsibility for the complete documentation of ongoing cases including maintaining statistics
- Networking with advisory centres such as Pro Senectute, Cancer League, Rehab and Social Psychiatric Services
Your profile
- CAS in Case Management or equivalent training in the field of Case Management is an advantage, or willingness to complete this
- Independent and structured way of working
- Motivated, honest, team-oriented and friendly personality with a high sense of responsibility
- High social and personal competence
- Medical expertise or many years of experience in healthcare is an advantage
- Proficient PC user skills
- Very good German language skills, spoken and written
