To complete our teams, the Fondation Soins Lausanne is looking for immediately or by agreement several:
Lead Nurse (M/F)
Activity rate between 80% and 100%
Permanent contract - daytime hours and some weekends
Your mission
- Support clients in their home life project with a partnership approach by giving meaning and coherence in close collaboration with their entourage and relatives while developing trusting relationships
- Manage the comprehensive and integrative care of clients relying on internal stakeholders and interdisciplinary teams and in good coordination with institutions involved in the care network
- Develop the therapeutic project taking into account the complexity of care and adapt it according to progress
- Provide assessment, advice, examination and treatment services, as well as nursing and basic care for quality client care
- Actively contribute to the sustainable home care of clients through prevention and health promotion measures while considering individual needs with personalised care and promoting client autonomy
- Participate in development projects and working groups focused on continuous improvement of home care and support services
Your profile and skills
- Bachelor’s degree in nursing or equivalent qualification (foreign diploma recognised by the CRS)
- Experience in the medico-social field and strong interest in community health
- Autonomy and decision-making skills
- Ability to collaborate in networks and multidisciplinary teams
- Sense of responsibility and professional ethics
