You have experience in Spitex and enjoy taking responsibility? For a modern Spitex organisation, we are looking for a committed Dipl. Nursing Specialist HF/FH with experience in case management. You can expect a varied role with a lot of personal responsibility and the opportunity to actively shape the care of your clients.
Your Tasks
- Holistic care and support of clients in their home environment
- Taking over case management including needs assessments as well as care planning and evaluation
- Coordination of interprofessional collaboration with general practitioners, hospitals, therapists, and other stakeholders
- Advising and supporting clients and their relatives
- Ensuring high-quality and resource-oriented care
- Proper electronic nursing documentation
- Contributing to the further development of care processes and quality standards
Our Requirements
- Completed training as a Dipl. Nursing Specialist HF/FH or equivalent qualification recognised in Switzerland
- Several years of professional experience in Spitex
- Experience in case management and needs assessments (RAI-HC advantageous)
- Independent, structured, and responsible working style
- High social competence and strong communication skills
- Driving licence category B
- Very good German language skills, both spoken and written
