About the job
The Réseau Santé la Côte (RSLC) is one of the four Vaud care networks according to the Cantonal Law on Care Networks (LRS). It brings together regional care providers (hospitals, nursing homes, medical-social centres, doctors, etc.) and municipalities. The network's mission is to improve, in close collaboration with its members, the coordination of care and the guidance of users within the healthcare system.
RSLC manages the Regional Information and Orientation Office (BRIO), whose missions include guiding people with loss of autonomy upon hospital discharge and managing and evaluating requests for nursing home placements.
To complement its social offer to its "Hospitals" partners, Réseau Santé la Côte is seeking a social worker at 50% on a permanent contract.
Within the BRIO team, in collaboration with social workers dedicated to nursing home placements and medical practices, you will offer your expertise to clarify social situations arising in the hospital. Your approach, experience, ability to build connections and refer cases to appropriate services enable vulnerable people to clarify their social situation and be supported throughout their care pathway.
Your main activities, with the RSLC acute care hospitals, include:
- Ensuring the processing of social and insurance-related requests that must be handled during hospitalisation in collaboration with caregivers (including liaison services).
- Where appropriate, directing the patient to the competent community social services.
- Being a resource person for social questions for hospital services.
Your profile:
- Degree in social work (Bachelor of Arts HES-SO in social work) or equivalent training.
- CAS (in mental health and social psychiatry, interprofessional addictions, or social management) – an asset.
- At least 2 years’ experience in the healthcare sector (hospital, extra-hospital, and/or nursing home and/or medico-social institution), as well as network work.
- Very good knowledge of social insurance, the Vaud medico-social system, and procedures related to the Justice of the Peace.
- Good writing skills and proficiency with common IT tools.
- Proven ability to work in a multidisciplinary team and network.
- Proactivity in the field of competence.
- Negotiation skills, listening, and empathy.
- Vehicle and driving licence required.
