Founded in Lausanne in 1843, the Asile des aveugles Foundation includes the Jules-Gonin Ophthalmic Hospital, a university ophthalmology service, a research centre, the Educational Centre for Visually Impaired Pupils (CPHV), social services, rehabilitation and low vision, and the Clair-Soleil medico-social establishment.
More than 600 employees mobilise every day in the service of visual health to offer diagnostic, treatment and support services to people with visual impairments.
To complete our team, we are looking for a
Coordinator of the Visual Rehabilitation Care Pathway
Your mission
- Receive and analyse requests addressed to the visual rehabilitation unit, verify admission criteria and prioritise interventions
- Carry out a comprehensive assessment of medical, functional, social and environmental needs and correlate them with the beneficiary’s priority needs
- Direct beneficiaries to appropriate internal or external services
- Coordinate interventions with stakeholders and ensure follow-up of the support plan
- Inform, advise and support beneficiaries and their relatives
- Facilitate communication between professionals and partners involved
- Ensure the quality and traceability of documentation
- Manage the administrative follow-up of the care pathway
- Contribute to the improvement of practices, tools and services of the department
Your assets
- Bachelor’s degree from a University of Applied Sciences or university in the health field, or an equivalent qualification
- Recognition of the diploma by the Swiss Red Cross is mandatory
- Several years of experience in the health, social or medico-social field, ideally five years, in Switzerland
- Experience in visual impairment, disability or functional rehabilitation is desirable
- Additional training in visual rehabilitation or care pathway coordination: an advantage
- Good knowledge of the medico-social network and social insurance in Switzerland
- Excellent command of French and ease with IT tools
