Reference Nurse (M/F) in CMS

-Bachelor in nursing or equivalent qualification (foreign diploma recognised by the CRS)

• Experience in the medico-social field, with a strong interest in community health
• Ability to work independently and make appropriate decisions
• Aptitude to collaborate within an interdisciplinary network and multidisciplinary team
• Sense of responsibility, rigour and respect for professional ethics

• Support patients in their home life project, in collaboration with their relatives and the care network
• Coordinate overall care with interdisciplinary teams and health partners
• Develop and adapt the therapeutic project according to evolving needs
• Carry out assessments, nursing care and necessary basic care
• Promote home care, prevention, health promotion and patient autonomy
• Participate in the continuous improvement of services and development projects
