The Ensemble Hospitalier de la Côte — EHC is a healthcare network at your service for over 25 years. It brings together establishments with complementary missions of acute care, rehabilitation, EMS accommodation, and primary care medicine, offering more than 40 medical disciplines within a region stretching from the shores of Lake Geneva in Ecublens to Gilly, and right into the heart of the Jura.
Our three EMS, two located in Morges and one in Aubonne, provide personalised, professional, and warm care to more than 180 residents. They also benefit from Temporary Care Centres that offer personalised daytime support and a lively social life promoting the well-being of elderly people. To strengthen our teams at the Temporary Care Centres (CAT) of Parc de Beausobre, Nelty de Beausobre, and Les Clos d’Aubonne, we are looking for a:
Reference Nurse for the CAT (M/F) at 60%
Permanent contract, starting as soon as possible or by agreement.

YOU HAVE
- A Bachelor of Science in Nursing or a nurse diploma recognised by the CRS.
- Successful experience in EMS and/or CMS, essential for this role.
- Training in Montessori and/or Humanitude, an advantage.
- Great autonomy, nursing leadership, and a good sense of initiative.
- Strong skills in clinical and social assessment.
- A sense of interdisciplinary teamwork and ease in collaborating with network partners.
- Good organisational skills and real administrative ease.
- Mastery of OPAS assessments, an asset.
- Good knowledge of the regional medico-social network.
- A driving licence and a vehicle.
YOUR OBJECTIVES
- Systematically identify the needs and resources of the beneficiary within a comprehensive clinical and social approach.
- Use your nursing leadership to ensure the role of CAT reference and coordinate beneficiary support.
- Develop, implement, and reassess the support plan with the beneficiary and their relatives, taking into account their life project and home care maintenance.
- Work closely with the CAT social workers and interprofessional teams.
- Coordinate complex situations in partnership with CMS, attending physicians, and other actors in the medico-social network.
- Decide and coordinate necessary actions with the beneficiary, their relatives, and the interdisciplinary team according to defined objectives.
- Contribute to prevention, identification of vulnerabilities, and anticipation of situations likely to compromise home care.
- Support informal carers, identify their needs and resources, prevent burnout situations, and promote appropriate relief.
