Lead Nurse 80-100%

Your tasks:

• Conducting needs assessments with various clients
• Nursing care and health checks
• Coordinating interventions with the planning team
• Regular monitoring visits to clients
• Supervision and follow-up of auxiliary staff
• Communicating information to various stakeholders, updating records
• Interdisciplinary cooperation with health insurance funds, doctors, hospitals
• Participation in conferences
• Regular on-call duty

Your profile:

• HES nursing diploma or equivalent with Red Cross recognition and right to practise
• Experience in home care and knowledge of InterRAI HC
• Proficiency in IT and mobile tools
• Ability to organise independently and flexibility
• Sense of responsibility
• Fluency in French; knowledge of German is an advantage
• Willingness to undertake on-call duty some evenings and weekends
• Driving licence and residence in the canton of Fribourg
