Job Description
Day shifts from Monday to Friday including public holidays
Independent, competent and resource-oriented care of our clients and their relatives in dealing with dementia diseases in their familiar living environment
Taking over case management for a defined client group
Planning, controlling and monitoring the care process
Supporting our clients in a self-determined everyday life at home by promoting independence and quality of life
Interdisciplinary collaboration with relatives, doctors and other services
Conducting assessments and status discussions with RAI-HC
Requirements Profile
Completed training as a qualified nurse HF
Professional experience in gerontopsychiatric care
Additional training in the field of dementia is an advantage (or willingness to further train in this area)
An independent, responsible way of working
High sensitivity and empathy
Willingness to travel by car
