Dipl. Nurse HF/FH with Case Management (80–100 %) | Spitex

As a qualified nurse with case management, you take on a central role with us. You manage care processes, coordinate demanding care situations and, together with your team, ensure professional and high-quality care for our clients in their familiar home environment.

You work independently, apply your expertise purposefully and receive the necessary scope for design to actively shape care processes.

Your main tasks include:

• Taking over case management including needs assessment using interRAI HC as well as planning, implementation and evaluation of care processes
• Ensuring professional, individual and client-oriented care in the home environment
• Coordination and interprofessional collaboration with general practitioners, relatives, institutions and other professionals
• Professional support and guidance of your team colleagues in complex care situations
• Professional care documentation as well as active participation in quality assurance and development

You bring with you:

• Diploma as a nurse HF/FH or an equivalent qualification recognised in Switzerland
• Professional experience as a qualified nurse; experience in the Spitex environment is an advantage
• Enjoyment of independent and responsible work as well as coordination of complex care situations
• Organisational skills, communication strength and enjoyment of interprofessional collaboration
• Good IT user skills and openness to digital work tools
• Driving licence category B or willingness to visit our clients in all weather conditions by e-bike
