The Bienne Hospital Centre is the public medical centre for the bilingual Bienne-Seeland-Jura Bernois region (Switzerland). Our case management team needs reinforcement. We are looking, from 1 November 2026 or by agreement, for a:
Case Management Collaborator, 80% - 90%
Your tasks – our position
- Assessment of needs in complex patient situations (evaluations)
- Communication with patients and their relatives regarding the planning and organisation of care solutions after hospital discharge
- Collaboration with Spitex organisations, rehabilitation clinics, nursing homes and medico-social institutions, as well as other institutions, to manage and organise targeted patient follow-up
- Obtaining financial coverage guarantees for follow-up care in hospital settings
- Helping care teams to respect the optimal hospital stay duration by clarifying and managing the hospital stay process and discharge planning from the outset
- Interdisciplinary collaboration with care services, specialised nurses, doctors and clinic secretariats
Your skills – our expectations
- Completed training in the health sector as a nurse ES/HES, with professional experience in a hospital environment
- Organisational skills, flexibility and stress resistance, service orientation
- Good communication skills, confidence and ability to think holistically
- Good computer skills
- Proficiency in German or French (spoken and written), with good knowledge of the other language or willingness to learn it
- Committed, motivated and proactive personality, capable of making a decisive contribution to the development of our organisation in a constantly evolving environment
