About the job
Your Tasks
- Advising and supporting patients with chronic, incurable and advanced diseases as well as their relatives within the interprofessional team
- Supporting the treatment teams of all wards with distressing symptoms and evaluating the measures taken
- Assisting with goal clarification (Goals of Care), decision-making and advance directives
- Participating in and shaping roundtable discussions as well as interprofessional meetings
- Supporting the organisation and coordination of follow-up solutions as well as documentation (consultation reports, ward round entries, action plans)
Your Profile
- Completed professional training as a registered nurse HF or FH
- Further training in palliative care (at least B2) or willingness to undertake it
- Enjoyment and confidence in advising and evaluating palliative care and treatment situations
- High communication skills for conversations with people in complex life situations
- Willingness to work in an interdisciplinary team and
- Confidence in communication with other professional groups
