Your tasks
- You take charge of the care and support process for your assigned clients
- You are responsible for case management and conduct the needs assessment using interRAI HC
- You take responsibility for implementing the nursing diagnostic process
- You are an important professional contact for our employees and support them with professional questions
- You assist with complex care situations and wound treatments
- Together with the team, you ensure a high and consistent quality of care
- You work closely and professionally with relatives, doctors, therapists, and other specialist agencies
- You document your services professionally and reliably

What you bring with you
- Diploma as a Nurse / Nursing Specialist HF, DN II, AKP or a tertiary-level qualification recognised by the SRK
- User knowledge of Perigon and interRAI HC is an advantage
- High social and professional competence
- Independent, reliable, and responsible working style
- Enjoyment of working in an interprofessional environment
- Very good oral and written German skills
- Driving licence category B
