Together with all professionals involved in the care of patients, we are responsible for effective discharge management and play an important role in the recovery process as aftercare and social counselling.
This is who we are
Your Tasks
- Professional advice and support for patients and their relatives in the area of psychosocial consequences of illness/accident/pregnancy as well as social law issues
- Organising follow-up solutions in the form of rehabilitation, nursing homes, convalescent cures, detox clinics and more
- Networking with further counselling centres
- Administrative tasks such as record keeping, correspondence, etc.
- Participation in interdisciplinary meetings, collaboration with external organisations
- Independent management of patient files
Your Profile
- You are currently studying Social Work at ZHAW and are looking for a second internship position
- You are an open, flexible and well-structured person who values working in interdisciplinary teams and collaborating with external counselling centres and institutions
- You enjoy communicating and are committed to finding the best possible solutions for patients and their relatives within the framework of discharge management
- You are used to working efficiently and keep a cool head even in stressful situations
