Contribute
- Ensure efficient, needs-based and sustainable discharge and transfer planning with the aim of guaranteeing quality of care and avoiding unnecessary readmissions and rehospitalisations
- Holistic case management of patients during their inpatient stay, taking into account their individual social, medical and care-related situation
- Lead and coordinate interdisciplinary discussions with patients, relatives as well as internal and external specialists
- Plan, organise and coordinate transfers to rehabilitation, convalescence, care institutions or home care
- Networking and collaboration with internal and external service providers, authorities, insurers and other involved parties
Bring along
- Training in health, social or a related field, ideally with further education in case management
- Professional experience in healthcare as well as experience in interprofessional collaboration
- Experience in advising and supporting people in stressful life situations
- Strong communication, coordination and organisational skills
- Independent, solution-oriented and networked way of working
- Knowledge in the area of social insurance
