For an innovative care organisation in the field of home healthcare, we are looking for a committed and independent certified nursing specialist HF/FH who enjoys competently advising and supporting people and families in their own homes.

Your Tasks
- You conduct initial interviews and needs assessments with clients and their relatives on site.
- You analyse the individual care situation and develop appropriate support options.
- You competently advise families and accompany them in decision-making.
- You carry out nursing assessments and initiate necessary measures in a structured manner.
- You carefully prepare care-relevant documents and prescriptions.
- You coordinate with internal specialists as well as external partner organisations.
- You ensure complete and professional documentation.
- You handle administrative tasks flexibly and independently from the home office.
Our Requirements
- Completed training as a certified nursing specialist HF/FH.
- Professional experience in outpatient care or Spitex environment.
- Strong communication skills and enjoyment of consultation discussions.
- Confident demeanour and ability to build trust with clients and relatives.
- Good clinical assessment skills and a structured way of working.
- Independent, solution-oriented, and responsible personality.
- Enjoyment of a dynamic environment with short decision-making paths.
- Good IT user skills and openness to digital working tools.
- Driving licence category B.
