As a home care nurse, you will be responsible for providing personalised care to patients directly in their living environment, ensuring quality, safe, and appropriate care for each situation. You will work with people of all ages, in collaboration with families, doctors, and various healthcare professionals.
Your main duties will include:
- Providing nursing care at home according to medical prescriptions and the specific needs of patients.
- Performing technical care such as dressings, injections, blood sampling, clinical monitoring, and other nursing procedures.
- Managing and monitoring medication treatments to ensure proper administration and effectiveness.
- Assessing patients’ health status, detecting clinical changes, and adapting care accordingly.
- Supporting patients in maintaining their autonomy and promoting their quality of life at home.
- Ensuring clear and compassionate communication with patients, their relatives, and care network participants.
- Participating in therapeutic education for patients and their families to help them better manage their health daily.
- Transmitting necessary information for patient follow-up and contributing to care coordination with all partners.
