Job Description
- Ensure comprehensive patient care to promote their autonomy at home
- Provide comfort, hygiene, and relational care to patients at home while respecting internal processes and procedures
- Deliver services related to preserving the patient’s autonomy and social life
- Maintain the patient care record and communicate information to the entire interdisciplinary team
- Apply clinical reasoning to optimise the quality of care and patient safety
- Be involved in the continuous improvement of practices
Qualifications
- Certificate as a Care and Support Assistant or equivalent training
- Experience in home care preferred
- Organised, adaptable, positive attitude
- Native French speaker or very good knowledge mandatory
- Foreign languages an asset
- Delegated Care training a plus
