A Reference Nurse for the Aubonne CMS

Mission

• Provide care and appropriate support to people of all ages, in various contexts, in collaboration with their relatives.
• Actively participate in a multidisciplinary manner in the organisation and monitoring of the care project defined with the client.

Profile

• Bachelor HES-SO in Nursing or equivalent, or foreign diploma recognised by the Swiss Red Cross
• Skills in managing a patient portfolio based on continuous assessment and monitoring of care objectives
• Knowledge of home care is an asset
• Ability to collaborate within a multidisciplinary team
• Listening skills and ability to provide support
• Ability to be proactive and position oneself as a partner in network work
• Ability to adapt to change, work autonomously, and manage complex and/or crisis situations
• Available, flexible and good adaptability
• Driving licence essential
