Social Worker for the North Adult Psychiatry Service
The North Adult Psychiatry and Psychotherapy Service (SPPAN) is seeking a social worker, with a workload between 90% and 100%, for its outpatient social unit (UPA) in Yverdon-les-Bains and for its mobile unit, as a case manager, starting 1 July 2026 or as agreed.
Context
The Lausanne University Hospital Centre (CHUV) is one of the five Swiss university hospitals. Thanks to its collaboration with the Faculty of Biology and Medicine of the University of Lausanne and EPFL, CHUV plays a leading role in medical care, medical research, and education.
The North Adult Psychiatry and Psychotherapy Service (SPPAN) is one of the clinical services of the CHUV Department of Psychiatry. It provides care for adult patients from the northern Vaud region, organised into hospital and outpatient centres.
The social service, integrated into these teams, is involved in developing individualised care plans, actively involving relatives and network partners to ensure comprehensive and coordinated support.
The position is based in two outpatient units:
- the outpatient psychiatry unit (UPA) in Yverdon-les-Bains
- the mobile psychiatry unit, as a case manager for people in crisis or vulnerable situations
These services work closely with patients and partners to ensure continuous and coordinated care.
Mission
At 40% at the UPA in Yverdon-les-Bains:
- You conduct psychosocial assessments and provide individualised follow-up
- You identify daily difficulties and co-develop solutions with the medical and nursing team
- You intervene in the field and work closely with the socio-health network (CSR, justice of the peace, institutions, etc.)
- You support and guide patients and their relatives in administrative, social, professional, and legal procedures
- You participate in the development of adapted life and housing projects
- You analyse and mobilise network resources to strengthen patient autonomy
- You work in synergy with a multidisciplinary team and network partners
At 60% within the mobile team (case management):
- You provide case management follow-up, promoting access to and continuity of care
- You meet patients and assess situations in the field, in collaboration with the psychiatrist
- You facilitate access to the care network by mobilising patient resources
- You support engagement in care and contribute to coordinated management
- You accompany care transitions (return home, post-hospitalisation, outpatient follow-up)
- You bring your expertise as a social worker and contribute to team reflections
- You analyse and mobilise existing networks to enhance coherence and effectiveness of interventions.
Profile
- You hold a HES-SO Bachelor’s degree in social work or an equivalent qualification (with SEFRI recognition for foreign diplomas)
- You have experience working with people suffering from mental disorders
- You have good knowledge of the Vaud socio-health network and medico-social, administrative, and legal structures
- You demonstrate analytical and synthesis skills
- You are autonomous, creative, proactive, and possess excellent interpersonal skills
- You hold a driving licence and have a vehicle (essential).
Depending on the profile, the salary scale will be between level 8 and level 9.
