Your Tasks
- Comprehensive care: You independently and resource-orientedly take on basic and treatment care of clients of all ages with diverse medical conditions.
- Responsibility in case management: In close collaboration with a qualified nursing professional HF, you are responsible for case management for a fixed client base.
- Process co-design: You actively participate in the planning, implementation, and optimisation of the care process.
- Promotion of autonomy: You support clients in organising their daily lives as independently as possible in their own homes for as long as possible.
- Interface management: You maintain professional interdisciplinary collaboration with relatives, doctors, and other involved services.
Our Requirements
- Completed vocational training as a Health Specialist EFZ
- An independent, responsible working style
- Openness, flexibility, and empathy
- Willingness to travel by e-bike or car
