Spitex am Kohlfirst ensures that people in the communities of Uhwiesen, Dachsen, Flurlingen and Feuerthalen-Langwiesen receive the care and support they need at home – professionally, warmly and reliably. As demand is steadily increasing, we need reinforcement.

Your job with us

• You accompany mentally ill people of all ages at home.
• You ensure professional psychiatric and psychosocial care.
• You conduct needs assessments with interRAI-CMH.
• You advise and guide relatives and clients.
• You ensure follow-up care after hospital stays.
• You support people in crises and through difficult phases of life.
• You are committed to building long-term nursing therapeutic relationships.
• You are responsible for the independent implementation and autonomous planning of the holistic care process, including psychiatric case management.
• You work together with external specialist and counselling centres.
• You provide specialist support in the field of psychiatry for the entire nursing team.
• You are part of the team that supports nursing students HF / FH.

What you bring

• Completed nursing studies HF / FH specialising in psychiatry (at least two years of professional experience in inpatient or day-patient psychiatric care).
• Enjoyment of independent, responsible and team-oriented working methods.
• High resilience and flexibility.
• Competence in self-organisation and self-reflection.
• Good knowledge of modern IT (including RAI-CMH or willingness to undertake further training).
• Driving licence category B (own car advantageous).
