Social Worker 60-90% - Acute Hospital

Exciting challenges await you

• Advising, caring for, and supporting patients and their relatives in sometimes difficult life situations
• Arranging and organising medically prescribed rehabilitation and convalescence stays
• Supporting, advising, and organising transfers to retirement and nursing homes
• Advising and supporting the planning of outpatient care at home (e.g. Spitex, meal delivery services, etc.)
• Arranging and advising on facilities and services in the areas of social, addiction, and disability support
• Coordinating with relevant external specialist agencies, authorities, and municipalities
• Participating in interdisciplinary reports, ward rounds, working groups, and projects
• Planning and organising patient transports
• Recording patient statistics within your area of responsibility

What distinguishes you for the job

• Specific education and training in social work/healthcare; relevant professional experience in a comparable institution in the health or social sector is an advantage
• Good knowledge of relevant issues in health, accident, and social insurance law
• Empathetic, balanced, and resilient personality with strong social skills
• Good oral and written communication skills; confident in German, both spoken and written
• Independent, responsible, and structured way of working
• Good administrative and IT skills
