We are looking for the CMS of Payerne:

Mission :

• Ensure all social follow-up at the home of people with physical, psychological dependency or a disability.
• Act as the "situation reference" for clients and assess their needs.
• Actively participate in a multidisciplinary way in the organisation and follow-up of the care project defined with the client.

Profile :

• Holder of a Bachelor in social work with a social service orientation or an equivalent qualification.
• Experience, if possible, in the social or medico-social field.
• Good knowledge of social insurance and the Vaud medico-social network.
• Able to work autonomously and in an interdisciplinary team.
• Good listening and negotiation skills.
• Holder of a driving licence and owning a vehicle (possibility to use a professional vehicle).
