- Outreach social counselling and support for adult patients and their social environment
- Inter-institutional networking (authorities, offices, residential facilities, outpatient services, guardians)
- Referral and triage tasks as well as work with relatives at home
- Participation in the further development of social counselling services
What you bring
- Degree from a university of applied sciences in Social Work (BSc)
- Competence in interprofessional collaboration and good communication and social skills
- High personal responsibility and an independent working style
- Professional experience with mentally ill people
- Resilient and flexible personality with an interest in issues concerning mentally ill people
- Driving licence category B
