About the job
The person with dementia and their environment are supported by a small care team. You work as a Dementia Coach in a team where you are responsible for holistic care and support. Coordination and advice for the care network is your core area.
You are an initiative, self-reliant professional who enjoys taking responsibility. You take pleasure in your profession and in dealing with people with dementia and their care network.
You hold a diploma and have training as a Dementia Coach or are willing to complete it.
You possess a category B driving licence and, if possible, your own car.
