For this key position, we are looking for a reflective and empathetic personality who effectively connects theory and practice.
Your tasks
- Professional leadership, support and empowerment of the team in complex nursing situations
- Main responsibility for nursing needs assessment with RAI LTCF
- Process responsibility for admission and discharge management
- Assess clinical situations using assessments and derive appropriate measures
- Promote and ensure evidence-based nursing interventions
- Evaluate nursing quality based on medical quality indicators, derive measures and implement improvements
- Plan and conduct case discussions
Your profile
- Completed studies as a nursing specialist HF / FH
- Supervisor RAI LTCF or interest in completing it
- Professional experience in long-term care
- Interest in interprofessional collaboration
- Clear communication and assertiveness
