Dipl. Psychiatric Nurse HF / FH – or with Experience in Psychiatry 20 – 40%

Start date by arrangement

Your area of responsibility:

• Psychiatric and psychosocial care and support of chronically and mentally ill people at home
• You conduct needs assessments with interRAI-CMH and take on case management
• Independent implementation and planning of the holistic care process
• You work independently in client assignments but are connected with referrers, psychiatrists/psychologists and general practitioners.

What you bring:

• Diploma in Nursing HF / FH with a focus on Psychiatry or professional experience in inpatient psychiatric care
• Independent, responsible and team-oriented working style
• Reliability and flexibility
• Driving licence category B, own car an advantage
