We are looking for the CMS of Avenches, a

Mission:

• Ensure the monitoring of the client’s situation presenting a medico-social health issue at home.
• Manage their care project by organising interventions on an interdisciplinary basis and in partnership with the client, their family and their doctor.

Profile:

• Hold a HES nursing diploma or equivalent and have recent professional experience in a hospital setting.
• Have a reflective practice with analytical and synthesis skills.
• Have good mastery of organisation and time management.
• Be comfortable in communication and negotiation situations with various partners.
• Be proficient with computer tools.
• Hold a driving licence and own a vehicle (possibility to use a professional vehicle).
