The Saphir Foundation, a multi-mission and multi-site foundation (psychogeriatrics, geriatrics, adult psychiatry), is active in the medico-social field in the canton of Vaud and in the reception and support of people weakened in their physical and mental integrity during their life process.
We are looking for our establishment EMS Mont-Riant in Yverdon-les-Bains, which accommodates 56 people and specialises in the field of advanced age psychiatry:

Whatever their role, the professionals of the Saphir Foundation share a common goal, that of supporting people daily to enable them to concretely experience the institutional service strategy "like at home". That is to say, to preserve their habits as much as possible, to offer a response adapted to their wishes and, through genuine interprofessional collaboration, to enable the well-being and happiness of the people.
As a Community Care and Health Assistant, you support people in daily living activities and contribute to their overall care, taking into account their physical, psychological, social and spiritual needs. You promote their autonomy and quality of life, while ensuring they are offered a safe, caring and adapted environment.
You provide care and delegated medical acts within your role, under the responsibility of the qualified nurse. You observe the health status and needs of the people, relay relevant information and contribute to the evaluation and adaptation of their care.
You participate in the development, implementation and monitoring of care plans in collaboration with the interprofessional team. You also contribute to the quality of handovers and continuity of care. Depending on needs, you participate in the reception and integration of new professionals, apprentices and trainees in the care service.
YOUR BACKGROUND - YOUR QUALITIES AND SKILLS
- You hold a Federal VET Diploma as a Community Care and Health Assistant (ASSC).
- You ideally have experience in supporting elderly people, particularly in the field of psychogeriatrics.
- You are comfortable performing care and delegated medical acts within your role.
- You adopt an approach centred on the well-being, needs and resources of the person.
- You demonstrate dynamism, creativity, initiative and enjoy teamwork.
- You are flexible, adaptable, organised and able to manage priorities.
- You have good interpersonal skills and know how to communicate appropriately with people, their relatives and professionals.
- You appreciate interprofessional collaboration and are able to question yourself in a continuous improvement approach.
- You have excellent command of French (spoken and written) and are comfortable with common IT tools, notably Carefolio.
