What you will achieve with us
- Advising, nursing and supporting people with a severe progressive illness in their home environment
- Independently assessing unstable and complex care situations
- Treating complaints (symptom management)
- Independently planning and organising assignments
- Building a sustainable care team at home and advising relatives and close ones
- Coordinating collaboration with external specialist agencies and professionals
- Conducting evaluations and adjusting electronic documentation
- Contributing to quality assurance and further development of care processes
- Working in the Mobile Palliative Care Team
What you bring
- Several years of experience as a nurse HF/FH
- Training or further education in Palliative Care / Oncology Nursing Level B2 or CAS
- High professional and social competence as well as good self-assessment
- Appreciative communication
- Skilled IT user knowledge
- Willingness to travel by bicycle
- Willingness to work day and evening on-call shifts
