Your Tasks

• Independent care and support of clients in the home environment
• Case management responsibility for a defined customer base including care planning
• Coordination and organisation of assignments in close collaboration with the team
• Interprofessional collaboration with general practitioners, therapists and other specialist agencies
• Careful documentation of services provided according to internal standards

Our Requirements

• Completed training as a qualified nursing specialist HF/FH
• Experience in outpatient care / Spitex is an advantage
• Independent, well-organised and responsible working style
• High professional and social competence, enjoyment of direct customer contact
• Confident IT user skills (MS Office; industry solution is an advantage) and driving licence category B
