Job Description

• Holistic care and support of guests in accordance with the care philosophy and care process

• Assuming tasks as the primary care contact person

• Responsibility and management of the care process for assigned guests, including RAI care needs assessment

• Electronic care documentation using CareCoach

• Planning and accompanying medical visits

• Assuming daily responsibility with coordination of the day team

• Leading case discussions and care reports

Requirements Profile

• Completed training as a Specialist in Long-term Care and Support FA

• Professional experience in long-term care is an advantage

• RAI knowledge desired or willingness to attend these courses (paid for)

• A warm and humorous basic attitude

• Enjoyment of diverse challenges

• Good German language skills, spoken and written
